Introduction:
In January, NorthCare Network, the Prepaid Inpatient Health Plan for Medicaid Specialty Services and Supports for Michigan's Upper Peninsula submitted a Notice of Intent to Apply (NOIA) to the Michigan Department of Community Health to serve as the Prepaid Inpatient Health Plan for the same area under the newly configured plan for Michigan which would reduce the number of PIHP regions from eighteen to ten beginning in January of 2014.
On February 6 we received the instructions for submission of the formal Application for Participation (AFP). The deadline for the initial submission was April 1. Many people have worked tirelessly in February and March to prepare the enclosed document(s) which were submitted on March 29.
We will continue to provide additional information to the Department until July 1 according to our workplan and requirements of the application. At this point that process continues and we are busily gathering the required additional information and will soon be in dialogue with the Department regarding our application as they begin the process of review.
We are pleased with our submission to date and want to express our sincere appreciation to the staff, community stakeholders, Board members, consumers and families who assisted with the process and provided valuable input for the submission. We will continue to add information to the website as we update the application.

The Survey was administered to individuals who were receiving at least one service in addition to case management. This Report provides a summary of results based on data submitted by June 30, 2012.
